Vital Records (Births and Deaths)

The Town Clerk’s Office has vital records for the Town of Holbrook from 1872 to the present. 

  • BIRTH RECORDS are available to any person whose parents resided in Holbrook at the time of the child’s birth.  Birth records are public records except when the parents are not married at the time of the child’s birth.  In such cases, a photo ID must be provided and only the individuals listed on the certificate can obtain the record.
  • DEATH RECORDS are available for any person who passed away in Holbrook or was a resident of Holbrook at the time of death.
  • MARRIAGE RECORDS are available for couples who filed their marriage intention in the Town of Holbrook.  

A certified copy of a birth, death or marriage certificate can be requested in person at the Clerk’s Office during regular business hours.  To expedite, please call the Clerk’s Office at 781-767-4314 and the certificate will be ready upon arrival. 

To request a certified certificate by mail, please use this Vital Records Request Form and mail the request to the address at the top of the document.  Please be sure to include payment as well as a self-addressed, stamped envelope or your request will not be processed.

The fee for all vital records is $10.00 per certified copy. Cash or check are accepted.
